Serves the age population of 18+

The Homeless and Housing Full Service Partnership Team (H-FSP4) is primarily field-based and works with DMH & Outreach Teams that refer clients to our program. The team provides mental health and substance abuse services, maintains housing vouchers and obtains housing using an integrated and holistic approach.

The population of individuals served by the FSP4 Team is seriously mentally ill, homeless and many have substance abuse disorders. 

Mental health services provided include assessment, therapy, targeted case management, family support, crisis intervention and medication support. Other services include housing placement and retention, benefits acquisition, and support for employment and educational services.

The FSP4 Team works in Downtown (Skid Row and adjacent neighborhoods), Boyle Heights (LAC+USC Medical Center and adjacent programs) and South Los Angeles (Vermont/Manchester vicinity and adjacent neighborhoods).
