The Crisis Transition Teams (CTT) are developed under SB82 to augment the services of the Exodus Recovery Urgent Care Centers (UCC) by providing linkage and short-term mental health services to special needs populations who require additional supports upon leaving the UCCs. The Crisis Transition Teams are located in each UCC and consist of a Crisis Transition Coordinator and a Crisis Transition Case Manager who will work together to link persons with mental health issues into the appropriate services needed to achieve recovery. These teams will provide intensive case management for up to 60 days following discharge from the UCCs to ensure stabilization, linkage to on-going services/supports and triage services within the consumers’ local communities.
