Requirements:
Three (3) years clinical experience in the health sciences related field with a minimum of one-year direct client care experience working with complex clients with SMI who may have multiple conditions and be high utilizers of the health system. Required experience with clinical EHR and DMH and/or DHS clinical documentation/platform(s). Valid CA driver’s license and good driving record is required. Must be 21 or over to drive. Spanish speaking preferred.
Job Summary:
Case Manager to provide comprehensive mental health services, case management and care coordination services to include assessment, care planning and crisis intervention for clients in Exodus contracted programs both in the field and at the program site. Staff must have experience with OP DMH documentation standards.
