Requirements:
MA required, (3) three years of clinical experience. Excellent therapeutic interventions, assessment, communication resources and crisis intervention skills.
Job Summary:
The individual will provide intensive case management and navigation for up to 60 days following discharge for clients. Interventions will be targeted to ensure continued crisis stabilization, linkage to ongoing services and support and triage services within the individual’s local community. Spanish speaking preferred.
